PARTICIPATION WAIVER
LIABILITY WAIVER, RELEASE OF CLAIMS & ASSUMPTION OF RISK
PLEASE READ CAREFULLY. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS.
In consideration of being permitted to participate in classes, workshops, use of tools or
equipment, use of the space, social events, or any other activities at the Center for People and
Craft (“Activities”), I agree as follows:
1. Assumption of Risk
I understand that the Activities involve inherent risks, including but not limited to cuts, burns,
falls, equipment failure, serious injury, permanent disability, or death. I voluntarily assume all
known and unknown risks associated with participation, whether caused by my actions, the
actions of others, or the condition of the premises or equipment.
2. Release of Liability
To the fullest extent permitted by Minnesota law, I release and forever discharge Center for
People and Craft, its owners, employees, volunteers, instructors, agents, and equipment
suppliers from any and all claims or causes of action for injury, illness, emotional distress,
property damage, or death arising out of or related to my participation in the Activities, including
claims based on negligence.
3. Indemnification
I agree to indemnify and hold harmless Center for People and Craft from any claims, damages,
losses, or expenses (including attorneys’ fees) arising from my actions or the actions of any
minor for whom I am signing.
4. Health & Conduct
I represent that I am physically able to participate, will not participate while impaired, will follow
all posted rules and instructions, and accept responsibility for my conduct and use of equipment.
5. Personal Property
Center for People and Craft is not responsible for lost, stolen, or damaged personal property
while on the premises.
6. Photo & Media Release I grant permission for photographs or recordings of me and/or my minor child to be used by
Center for People and Craft for promotional or educational purposes without compensation.
7. Minors
If signing on behalf of a minor, I represent that I am the parent or legal guardian and agree to all
terms on the minor’s behalf.
8. Miscellaneous
This agreement applies to all future visits and Activities. If any portion is found unenforceable,
the remainder will remain in effect. This agreement is governed by Minnesota law.
By signing below, I acknowledge that I have read, understand, and voluntarily agree to this
waiver and release of liability.
